COvid-19 SAFETY, PRECAUTIONS, AND ASSUMPTION OF RISK

Safety Precautions:
We have implemented the following preventative measures to keep our staff and clientele safe during our photo sessions.
• A reduction of the number of sessions each day to no more than 3 clients in a standard 8 hour day
• We recommend coming to your shoot alone unless:
     • You feel you need help from a friend that would be considered essential
     • You are a parent accompanying a minor to the session
     • You are being photographed as part of a couple or group
• Hand sanitizer will be readily available to everyone and surfaces within the studio will be sanitized after each session
• All appointments will observe the minimum 6ft distancing for the duration of the appointment, and masks will be worn by the staff at all times. Clients will remove their masks during the active photography session. 
• Physical contact will be and kept to a minimum and limted to props, clothing, hair, or any adjustments, and will be made with masks on. The only acception will be make up application and adjustments as necessary by make up artist, who will be wearing a mask at all times. 

Self-evaluation:
* I affirm that I haven’t had any of the below COVID-19 symptoms in the last 10 days:
* Fever or chills
* Cough
* Runny nose, nasal congestion or sneezing without other known cause
* Sore throat
* Difficulty breathing or swallowing
* Unexplained fatigue or malaise
* Headaches
* Gastrointestinal upset
* Decrease (or loss) of sense of taste or smell
* I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 10 days.
* I affirm that I, as well as all household members, have not traveled internationally within the past 10 days.
* I understand that Headshots321 and Truc Allen Media has implemented measures to help prevent the spread of COVID-19.
